For the past seven years I've had a similar problem with high trigs. I have a flowsheet with values going back to 4/4/2001 with a value of 1430 and the highest of all -- 3910 on 6/21/02. Most values after this date show a dramactic decrease from 940 for the next result to 247 for 9/9/08. 
 
In the beginning of this period and until 12/05 Kaletra was my PI. It was around this same time, perhaps in 2002, that I was also diagnosed with type 2 diabetes, and had to make some big changes in my diet and started to exercise -- not nearly as much as I really should, but I make it part of my routine now. I tried niacin, but discontinued its use. I switched from using any cheap oil to fry food to canola and or olive oils, started taking fish oil capsules, Tricor, and incorporating flax seed (sometimes as seeds and other times as meal), more beans, peanuts and other nuts, too in my diet, and in 12/5 switched to Aptivus for a PI. I can't think of any other changes that occurred in this time period that could relate.
 
My decrease is probably a combination of factors. Others may have similar or different experiences and insights from mine. That's the wonderful thing about people here.

I have been on a wide variety HIV meds since testing positive mid-
1995 and within a year or so starting having very high tri-glyceride
levels.

My triglycerides hover around 1500 which has become sort of "normal"
for me. Sometimes they have been higher and once got up to about
3000 at which point I developed pancreatitis and lactic acidosis.

Over the corse of time, I have been on tricor and niaspan to help
control them but nothing really works. My current doctor is
considering a different prescription medication. He says it's not
really optimal but is the only other option at the moment. Previous
doctors have told me any diet modification would be useless because
the numbers are so high.

My cholesterol is usually at or slightly under 200 and that has been
effectively managed over time first with Lipitor and now Crestor.

Has anyone had success combatting very high triglyceride levels? If
so, how were you able to do this?
Does anyone have resources I can go to regarding changing my diet,
alternate medications or therapies, etc?
